What does the Medical Issues request look like for patient in Triage?

If the Medical Issues option is enabled with myGP Triage, the patient will have the option to contact the practice via online services for several different subjects.

This is the user flow the patient will experience:

If a patient chooses the Medical Issue triage option, they will be brought to the following screen:

If they have chosen Yes for urgent medical issues, they will be presented with these screens:

If the patient has chosen No to urgent medical symptoms, they will have the following options:

By selecting Next, they will be brought first to the default COVID Screener:

After answering, they will brought to the final questionnaire pathway for Medical Issue request:

The request will be sent to practice as an email:

Please Note: the average time to deal with this type of request is 2 days.
