Getting your practices unique URL for web triage

To help your patients access your Triage solution, you will need to add your unique URL to your website

How to get a unique URL for your practice

You will need to add the unique 3-letter code for your practice to the URL below

https://request.mygp.com/

To get your 3-letter code:

• Go to your iPLATO Connect platform, and click on Settings>Pracetice information.

• Find the web address at the top of the screen

• The last 3 letters are your unique code for your practice

For example, this practice code is ifh.  So the full URL for this practice web based triage page is:

https://request.mygp.com/ifh
